In this Viewpoint, Gostin and colleagues review recommendations developed by experts convened by Georgetown University on how the international community can respond to the 2018 Ebola outbreak in the Democratic Republic of Congo (DRC) in the midst of active conflict and insecurity from armed rebels.
This JAMA Guide to Statistics and Methods explains the difference between fixed and random effects in treatment effect estimates, and the rationale for using random-effects meta-analysis to determine treatment effects across randomized trials conducted in heterogeneous patients and settings.
In this Viewpoint, Harlan Krumholz and colleagues discuss the promise that social media data hold for helping researchers better understand social determinants of health, and the challenges that must be overcome to reliably link social network data to clinical and health outcomes.
In this narrative medicine essay, a family practitioner tells the story of how her relationship with an old order Mennonite woman whose newborn son she examined and took to the hospital for cardiac surgery one Christmas day turns to friendship and a relationship with her broader community when the woman stays with her during her newborn daughter’s cardiac surgery.
A 58-year-old woman had a 2-week history of right knee, right ankle, and bilateral finger joint pain with a facial rash after a febrile illness in her daughter and preschool grandchildren, and an ANA level elevated to 1:160 (homogeneous pattern; negative <1:80 serum dilution. Twenty years ago, the Centers for Disease Control and Prevention (CDC) declared family planning and contraception services as 1 of 10 great public health achievements in the United States in the 20th century. Access to contraception is associated with reductions in teen pregnancy rates, improved health outcomes for women and their children, increased educational levels for women, enhanced economic security for women and their families, and other benefits that have led the US Agency for International... This Medical News article is an interview with Rebecca Thurston, PhD, about her recent study published in JAMA Internal Medicine that found sexual harassment and assault was associated with poorer midlife health in women.
This Viewpoint discusses the likelihood of Medicaid expansion in Maine, Wisconsin, Utah, and Kansas, where 2018 midterm voters elected candidates supportive of expansion or voted for it in ballot initiatives, and reviews evidence on associations between state Medicaid expansion and improved health outcomes for beneficiaries.
In this Viewpoint, Ioannidis and coauthors compare overuse of computed tomography and magnetic resonance image scanning with large-scale haphazard population screening for disease and call for dedicated investigations to understand how to reduce unnecessary and wasteful diagnostic imaging.
The Arts and Medicine feature reviews the film Beautiful Boy, a dramatization of journalist David Sheff’s 2008 memoir about his son’s descent into methamphetamine addiction and his tentative recovery.
Stroke is one of the most devastating complications of sickle cell anemia (SCA), and risk of stroke starts very early in life. Before screening techniques were available, 11% of children with homozygous SCA had ischemic stroke by their 20th birthday. Although most children with SCA will have neurologic recovery after stroke, approximately one-third will have long-term neurologic sequelae, including motor impairment. Moreover, children with SCA who have had a stroke often have neurocognitive impairment and difficulties with academic performance.
Reducing platelet activity with aspirin and other antiplatelet agents is an important factor in the prevention and management of atherothrombotic vascular events. For this indication, aspirin has both beneficial and potentially harmful effects; it can diminish or reverse thrombus formation (eg, in the setting of acute myocardial infarction or stroke), but it also increases the risk of bleeding.
More than 3 decades after its clinical use was first described, amifampridine has received approval for adults with rare, debilitating Lambert-Eaton myasthenic syndrome (LEMS).
A new type of diagnostic test to help detect cytomegalovirus infections in newborns within their first 3 weeks of life has received approval.
The FDA has cleared a noninvasive vagus nerve stimulator as the first device indicated to help prevent cluster headaches in adults.
Paleopathology, as we have pointed out, is a new word for a very old subject, which is only now beginning to attract the attention it deserves. All physicians interested in the scientific side of medicine, especially those who have professional and educational interests at heart, should be familiar with its recent progress. The term was originally proposed, we believe, by Ruffer in 1914, when, in the course of researches on ancient Egyptian mummies, he realized that the study of pathologic changes... Читать дальше...

To the Editor In the JAMA Insights Clinical Update on noninvasive prenatal screening using cfDNA, the authors discussed test failures that occur when the fraction of fetal DNA is too low, leading to a diagnostic test. The cited fetal fraction threshold of 4% is historical and modeled and follows empirical reasoning. However, a standard method for its calculation has not been set, and higher levels of detection for common aneuploidies at fetal fractions between 2% and 4% can be obtained using proprietary algorithms.
To the Editor The Viewpoint by Dr Ioannidis discussed the need for reform of nutritional epidemiologic research. We were surprised by the “nonexpert” calculations of gain or loss in life expectancy using data from our meta-analysis. We presented the findings in our meta-analysis as relative risks (RRs). Ioannidis calculated from those RRs the number of years of life gained or lost by consumption of a number of foods. Such calculations are not in line with statistical methodology and should not be... In Reply In response to our JAMA Insights article on noninvasive prenatal genetic screening, Drs Cohen and Westover advocate for universal public and private reimbursement for cfDNA tests, arguing that it will reduce disparities in access to prenatal screening. Approximately 40% of US pregnancies are supported by Medicaid. This population is, by definition, confined to the lowest end of the income scale and to women who cannot access other forms of health care coverage. These women are predominantly... Читать дальше...
