In Reply In response to the comments by Dr Koster and colleagues, the OPTIMAL trial enrolled patients aged 18 to 70 years, which is the age range of most patients undergoing adult cardiac bypass surgery in China. We agree that our study results, especially regarding seizures, should be interpreted with caution for patients older than 70 years.
In Reply Dr Fonseca raises the issue of potential confounding by low-dose aspirin in our study of the association between gout flare and cardiovascular events. While we did not adjust for low-dose aspirin specifically, our multivariable model included adjustment for current (≤60 days) or past (>60 days) prescription of any antiplatelet drug (ie, aspirin [tablet strength of ≤325 mg], dipyridamole, clopidogrel, prasugrel, ticlopidine, and ticagrelor) prior to the cardiovascular event or matched index date for controls. In Reply We thank Dr Kirkegaard and Ms Manaugh for raising the importance of expanding access to MOUD, especially among populations who are incarcerated and justice-involved. Expanding equitable access to MOUD is critical to address overdoses in these populations, given high rates of substance use disorders, overrepresentation of racial and ethnic minorities in the justice system, and the very high risk of drug overdose during reentry.
