In Reply Mr Xiao and colleagues raise 5 issues related to our trial of tirzepatide. First, the glucagon-like peptide-1 (GLP-1) receptor agonist class has been available for the treatment of type 2 diabetes since 2005 and for the treatment of obesity since 2014 and has been the subject of increasing worldwide use. Although nausea, diarrhea, constipation, and vomiting are the most common adverse effects of this class of medication, appropriate clinical treatment of patients can reduce these common adverse effects and increase tolerability. To the Editor As reported by Dr Aronne and colleagues, tirzepatide demonstrates therapeutic benefits in weight loss; adults with obesity completing the 36-week lead-in period of the trial experienced a mean weight reduction of 20.9%. From randomization at week 36, those who switched to receiving placebo had a 14% weight regain, while those continuing tirzepatide achieved an additional 5.5% weight reduction during the 52-week double-blind period. However, we have several concerns.

In Reply We agree with Drs Gallo and Santiago that research is needed to identify and target the upstream contributors to disparities in hospital readmissions. These root causes function both at the level of the hospital and outside of the hospital via the social determinants of health. In our recent study, we proposed a novel measure of equity—an important domain of health care quality that has only recently been incorporated into formal hospital accountability programs. Quality measurement, through... Читать дальше...
To the Editor A recent study that used Medicare administrative data reported disparities in readmission rates for Black compared with White beneficiaries and notable segregation in the care of Black and White patients among hospitals. These findings necessitate attention to additional structural factors contributing to the observed disparities, extending beyond between-hospital–level segregation to the community and other health system levels.

In Reply We thank Mr Lu and colleagues for their interest in our recent article. In response to their first comment, it is important to note that body weights in this study were self-reported by participants approximately every 2 years and were not measured at clinic visits. Therefore, the frequency of clinic visits did not dictate the availability of weight data or the ability to assess recent weight change prior to a cancer diagnosis. However, we agree that outside this study, frequency of clinic... Читать дальше...
