Race, Gender, and Donor Heart Acceptance—Reply

Jamanetwork.com

In Reply Ms Majeed and colleagues pose important questions regarding our study. First, the COVID-19 pandemic had several possible implications for transplant care. To explore these implications, we reexamined the number of donor heart offers until acceptance as in the original article but censored observations after February 29, 2020, to exclude the COVID-19 period. For both analyses, we maintained the start date of October 18, 2018, because the United Network for Organ Sharing changed heart transplant listing criteria on this date. Читать дальше...

Race, Gender, and Donor Heart Acceptance

Jamanetwork.com

To the Editor Cardiac transplant is a lengthy process informed by social determinants of health and race-based differences. A recent study revealed that Black candidates had lower cumulative incidence of heart offer acceptance by transplant center teams compared with White candidates, and offer acceptance was higher for women than for men.

Clinical Implications of New Drinking Water Regulation

The Valediction in Poetry and Medicine

Jamanetwork.com

Valedictions are essential in poetry (from those of Catullus to Donne and, in more recent times, from Seamus Heaney to Adrienne Rich), expressing as much devotion and renewal as they do sorrow and the elegiac. So at this difficult moment of transition in medicine, when many physicians are leaving the profession for a variety of distressing reasons, perhaps it’s not surprising to encounter a valedictory poem like “Doctor.” As the speaker in the poem cleans out an office, its “files stuffed with stories... Chelation Therapy Following MI

Jamanetwork.com

In 2013, JAMA published the results of the Trial to Assess Chelation Therapy (TACT), a randomized clinical trial, that caught the cardiology community by surprise. This trial suggested that chelation therapy, in which edetate disodium (EDTA) is infused to remove heavy metals such as cadmium and lead, may reduce the risk of major adverse cardiac events in patients following myocardial infarction. The benefit was greatest among patients with diabetes. There was no obvious mechanistic basis for the findings.

PSA Screening and Prostate Cancer Mortality—Reply

Jamanetwork.com

In Reply We thank Dr D’Amico for his Letter, in which he states that nonattendance and contamination may have led the CAP study to underestimate the benefit of an invitation to a single PSA screening test for reducing prostate cancer mortality over a median 15-year follow-up (intention-to-treat estimate rate ratio, 0.92 [95% CI, 0.85-0.99]; P = .03). This finding corresponded to a reduction of 1 fewer prostate cancer death per 1000 men invited to screening compared with the control group over a median... Читать дальше...

PSA Screening and Prostate Cancer Mortality

Jamanetwork.com

To the Editor In a secondary analysis of the Cluster Randomized Trial of PSA Testing for Prostate Cancer (CAP), the secondary end point of prostate cancer–specific mortality was significantly reduced, with a rate ratio of 0.92 (95% CI, 0.85-0.99; P = .03) after a median follow-up of 15 years. I agree with the authors that this result is modest, yet it is important to note that both nonattendance (ie, individuals randomized to the intervention group who did not have a prostate-specific antigen [PSA] test) and contamination (ie... Aeroplanes in Medical Service

Jamanetwork.com

Amid the rapid changes that spell the progress of our day, we tend to lose enthusiasm for the novelties that are invading modern life in astonishingly quick succession. Civilized man has become blasé. He has come to expect new discoveries and novel inventions at every turn; and, surfeited by their actual production, he reacts with surprise or gratitude only when some exceptional contribution is made. He is prone to forget the dramatic chapters of medicine that have included the evolution of anesthesia... Читать дальше...

Uterus Transplant—The Frontier of Innovative Fertility Treatment

Jamanetwork.com

For some, the study by Testa et al reads like science fiction. This seminal study represents a major step forward for patients with uterine-factor infertility. Uterus transplant is the only restorative treatment available to patients with a formerly dysfunctional or absent uterus, who can now, for the first time, carry their own biological child.

Error in Study of Antibiotics for Pediatric Sinusitis

Jamanetwork.com

To the Editor On behalf of my coauthors, I write to report an error in our Original Investigation “Treatment Failure and Adverse Events After Amoxicillin-Clavulanate vs Amoxicillin for Pediatric Acute Sinusitis,” which was published in the September 19, 2023, issue of JAMA. In this study, we compared amoxicillin-clavulanate vs amoxicillin for the treatment of acute sinusitis in a cohort of outpatient children and adolescents aged 17 years or younger with a new outpatient diagnosis of acute sinusitis... Error in Study of Antibiotic Treatment for Acute Sinusitis in Children and Adolescents

Jamanetwork.com

In the Original Investigation titled “Treatment Failure and Adverse Events After Amoxicillin-Clavulanate vs Amoxicillin for Pediatric Acute Sinusitis,” published in the September 19, 2023, issue of JAMA, incorrect data for treatment failure were reported, as described in the accompanying letter. An error occurred in how the component outcome of days with an antibiotic prescription without an outpatient encounter was captured. While only 1 component of the outcome definition was impacted by the error... Читать дальше...

Vaccines Lowered Risk of Long COVID in US Veterans, but Not Completely

Jamanetwork.com

While the incidence of persistent and recurring symptoms of SARS-CoV-2, also known as post–COVID-19 condition or long COVID, decreased over the course of the COVID-19 pandemic, the risk remained even among those who were vaccinated, according to a new study published in the New England Journal of Medicine.
