Pulmonary Vein Isolation and Left Atrial Appendage Ligation in Atrial Fibrillation—Reply

Jamanetwork.com

In Reply Drs Xia and Su raise several queries about our study, the aMAZE trial. First, they state, “The study did not assess left atrial appendage electrical isolation, which is important considering the mechanism of action of the intervention,” implying this affected outcomes. The proposed mechanism of action of LAA ligation as adjunctive therapy to PVI was 2-fold: LAA electrical isolation and left atrial volume (LAV) reduction. LAA ligation results in LAA ischemic necrosis, atrophy, and resorption, analogous to amputation. Читать дальше...

Pulmonary Vein Isolation and Left Atrial Appendage Ligation in Atrial Fibrillation

Jamanetwork.com

To the Editor A recent study investigated the safety and effectiveness of percutaneous left atrial appendage (LAA) ligation with catheter pulmonary vein isolation (PVI) for nonparoxysmal atrial fibrillation (AF). Although the study design and execution were commendable, we have several concerns about its methodology and interpretation. First, the study’s primary outcome measure of freedom from documented atrial arrhythmias at 12 months may not have fully captured the efficacy of percutaneous LAA ligation adjunctive to PVI isolation.
