The New York City police commissioner says an officer who fatally shot an emotionally disturbed woman wielding scissors and a baseball bat did not follow department procedures and has been placed on modified duty.

Police say the sergeant had persuaded the woman to drop the scissors but she then grabbed a baseball bat.
