During the open-top bus tour, a fan attempted to throw a ball to the South African star.
However, the woman misjudged her throw, and instead of getting the ball to Kolisi, she instead hit Rachel in the head with the ball.
After realising the result of her throw, the fan looked mortified over her mistake.
Rachel had to sit down after suffering the blow as those around checked on her.
Kolisi himself looked shocked as he offered out a hand to check that she was okay.
Rachel has since appeared to be in good spirits about the accident as she shared a clip on her Instagram story with a laughing emoji.
She also wrote: “You know when you trip in fall in public and you really hope no one saw it it was a great throw.”

One fan joked: “Can we protect the First Lady please.”
A second added: “Nah. Siya’s face makes this more funny.”
